This information may include, but is not limited to, academic, residential, achievement, performance, attendance, personal history, disciplinary, arrest, and conviction records. I hereby direct you to release such information upon request of the bearer. I understand that the information released is for official use by Rossville Christian Academy and may be disclosed to such third parties as necessary in the fulfillment of official responsibilities. I hereby release any individual, including record custodians and references, from any and all liability for damages of whatever kind or nature which may at any time result to me on account of compliance, or any attempts to comply, with this authorization."

Certification of Authenticity & Background Check Consent
I certify that the statements given on this application are true and complete to the best of my knowledge. I authorize the investigation of all statements and the contacting of references and/or all former employers as may be necessary in making the hiring decision and I hereby release them and the Rossville Christian Academy from all liability for any damage(s) resulting from the investigation.
I agree to release all investigative records to the Rossville Christian Academy for examination for the purpose of verifying the accuracy of criminal violation information as required by the Tennessee Code Annotated, Section 49-5-406(a)(1). If offered employment, I will supply a fingerprint sample for a criminal history records check to be conducted by Rossville Christian Academy.Knowingly falsifying information required by 49-5-406(c)(2) shall be sufficient grounds for termination of employment and shall also constitute a Class A misdemeanor which must be reported to the District Attorney General for prosecution. The accuracy of such information may be verified by fingerprint and criminal history check conducted by the TBI and FBI pursuant to 49-5-413(a)(2).
In the event of employment, I understand that false or misleading information given in my application or interview(s) may result in immediate discharge. I understand that I am to abide by all policies and procedures of the employer which may be changed from time to time at the employer's discretion. I understand, also, that I will agree to have payroll checks direct deposited.
Further, I understand that this application is NOT a contract of employment.
